Beginning Tuesday, October 13, an online health screening form will need to be submitted once a week for all students in the Mayfield Central School District.
Going forward, the online form will be emailed Monday mornings to all Jr/Sr High School students and to parents of all elementary school students. The link to the form also can be accessed through the homepage of the district website, www.mayfieldcsd.org.
The health screening will ask if the student is experiencing COVID-19 symptoms, as well as questions that address whether the student has traveled to certain areas or has been in contact with someone who has COVID-19. The form should be completed before the student leaves their home in the morning.
Students’ temperatures will continue to be checked daily by a district staff member before they enter the bus or school buildings.
