2025-26 Pre-K and Kindergarten Registration Open Feb. 12

Pre-K and Kindergarten Registration for the 2025-26 school year is now open. Registering your child for Pre-K or Kindergarten is an important step and the staff at Mayfield Elementary School believe this process should be an easy and enjoyable one for everyone.

If your child is already enrolled in the Pre-K program, you do not need to register them for Kindergarten.

Important dates:

    • Registration deadline: Friday, Mar. 28
    • Pre-K and Kindergarten Information Night: TBA
    • Possible In-District Lottery for Pre-K: Friday, April 4 – The district has availability for 36 children in Pre-K. If more than 36 children are registered, a lottery will be drawn on Friday, April 4, 2025.

Eligibility requirements:

    • Any child age 4 on or before Dec. 1, 2025 is eligible for Pre-Kindergarten for the 2025-26 school year.
    • Any child age 5 on or before Dec. 1, 2025 is eligible for Kindergarten for 2025-26 school year.
    • Residency within the Mayfield Central School District.
    • The New York State Department of Health has mandated that no pupil can be admitted to school without the following immunizations: List of Required Immunizations. The law requires certification of immunization records from either a physician, public health agency or medical facility.

Directions for registering a student:

Step 1:
Gather the following documents and forms:

    • Two proofs of residency. You will need one of each of the following options:
      • Utility bill (National Grid, Spectrum or fuel delivery)
      • Driver’s license with parents’ current address on it
      • Lease agreement/mortgage (in the name of registering parent
    • Student’s birth certificate (the original with parents’ names on it)
    • If student is not living with both biological parents – a copy of the custody agreement, or both parents need to show identification and sign registration form (in the presence of the registrar)
    • Records from previous school including academic and health/immunization
    • Immunization and Physical Records – Please contact your child’s doctor to send records or complete the following forms:  
      NYS Health Exam
      Immunizations & Lead Screening
    • Optional:  Dental Records – Please contact your child’s dentist to complete the following form:  Dental Health Certificate

Step 2:
Pick up a Registration Packet at Mayfield Elementary School, 80 N. Main St., Mayfield, NY 12117.
Forms are available for pick-up Wednesday, Feb. 12 – Friday, Feb. 21.

Step 3:
Once all forms are completed, please call the Elementary office to schedule an appointment to review your documents. Registration appointments are available during February break, Wednesday, Feb. 19 – Friday, Feb. 21. Appointments are also available Monday, Feb. 24 – Friday, Mar. 28.

Extenuating Circumstances:

If student lives in the district with an adult other than parent(s), the district needs legal court papers determining the custody of the student. A meeting with principal and/or superintendent may be necessary for approval. Pease bring any other extenuating circumstances to the district registrar.

Out-of-District Registrants:

If student lives out of the district and wishes to be considered for non-resident status, they must send a letter requesting attendance including the reason for the request to the superintendent.  Upon approval from the superintendent, the student may register proceed with the registration process.
